A. Wrong, numerous mucin-containing cystic spaces and mucous cells were evident throughout the lesion.
B. Wrong, although the stroma was highly hyalinized, there were numerous mucin-containing cystic spaces and mucous cells intermixed with epidermoid cells which supports the diagnosis of mucoepidermoid carcinoma.
C. Right, the lesion was formed of nests of epidermoid and mucous cells (red arrow). There were numerous mucin-containing cystic spaces along with psmmomma bodies (green arrows) and hyalinized stroma. Clear cell change was seen in many epidermoid cells (black arrows). MAML2 gene rearrangement is the classic molecular event in MEC. The clear cell change in MEC is caused by glycogen deposition. However, psmmomma bodies are rare to be found in MEC, stromal hyalinization isn’t uncommon. There is no specific grading system for central MEC, however most of these cases were low grade.
